Referrals

Referrals, Chronic Disease Management Plans, Team Care Arrangements and DVA D904's can be faxed to the office on 07 3036 5397 or sent via Medical Objects secure messaging service to Janine Hoult Occupational Therapist, Inlifetherapy.

Reporting requirements for Department of Human Services Medicare, Department of Veterans Affairs, regarding treatment cycles are routinely followed.

Some patients may be referred through another Occupational Therapist if they are a DVA client for an initial breath management assessment.

The following information would be greatly appreciated:

  • Details of client (name, DOB, address, phone for contact, Medicare or DVA number)
  • Referrers name, address and provider number
  • Reason for referral and medical history

When to Refer

  • Breath and recovery skill acquisition
  • To improve home access and falls prevention
  • To assist with reduced activity tolerance and mobility at home and in the community
  • Palliative breath management
  • Aids prescription for independence at home in personal and domestic daily tasks.

Presentation on Breathlessness, Case studies and Literature Review

Presentations have been done on the management of breathlessness for the Lung Foundation, General Medical Practices and Hospital Respiratory Specialists and Allied Health Professionals;

This is available in order to encourage networking in the sphere of breath and fatigue management with a goal of making a smooth transition for clients who use both hospital and community services to manage their chronic and complex conditions. A time can be arranged to coincide with your regular team meetings.

Breath Classes for Patients

Breath classes can be organised for medical practice clinic patients who are;

  • Querying independent treatment and want to know more or
  • Not able to attend larger pulmonary classes because of chronicity issues, or
  • As a precursor to attend an 8-12-week rehabilitation programs as an outpatient

These classes can be held at the practice so patients are familiar with the environment and feel more confident to attend what may be their first attendance at a health education class / group.

A breath class could be arranged as a one off or at designated intervals. A duration of 30 min could include the effect of disease on breathing, simple breath management tips and with use of a pre-evaluation form could target some specific questions.
